We are not a healthcare provider, insurance company, or medical advice service. We help you compare prices—we don't provide care or make coverage decisions.

You may use ClearCareHQ to: • Search and compare healthcare prices • View cash and insurance-based pricing • Find providers in your area You agree not to: • Use automated systems to scrape or bulk-download our data • Misrepresent the source of our data or your affiliation with us • Use our service for any unlawful purpose • Attempt to access our systems or data beyond normal use

The prices shown on ClearCareHQ come from hospital transparency files required by federal law. While we work to keep this data accurate and current, you should understand: 1. Prices may change. Hospitals update their files periodically. The price you see may differ from the current price. 2. Exact vs. estimated. We label prices as "Exact" (matched to your specific plan) or "Estimated" (based on similar plans). Estimates are approximations, not guarantees. 3. Additional costs may apply. Base procedure prices may not include all components (anesthesia, facility fees, etc.). Always confirm the full cost with your provider. 4. Verify before booking. For any significant procedure, call the provider's billing department to confirm your actual out-of-pocket cost. ClearCareHQ provides information to help you make decisions. We are not responsible for differences between displayed prices and actual costs.

ClearCareHQ does not provide medical advice. Our service shows price comparisons—it does not recommend treatments, providers, or courses of action. Always consult with qualified healthcare professionals about your medical needs. Price should be one factor in your decision, not the only factor.

We source data from publicly available hospital pricing files and work to present it accurately. However, we cannot guarantee: • Completeness of data (not all hospitals or procedures may be listed) • Timeliness (source files may be outdated) • Accuracy of source data (hospitals may publish incorrect information) If you find inaccurate data, please report it to us at data@clearcarehq.com.

The ClearCareHQ name, logo, website design, and original content are our intellectual property. You may not use our branding without permission. The underlying pricing data comes from public hospital files. Our value is in aggregating, processing, and presenting this data—not in owning the raw data itself.

ClearCareHQ is provided "as is" without warranties of any kind. We do not guarantee that our service will be uninterrupted, error-free, or meet your specific needs. To the extent permitted by law, we disclaim all warranties, express or implied, including warranties of merchantability and fitness for a particular purpose.

ClearCareHQ is a free informational tool. We are not liable for: • Decisions you make based on our data • Differences between displayed and actual prices • Costs incurred from medical procedures • Any indirect, consequential, or incidental damages Our total liability, if any, is limited to the amount you paid us for the service (which is $0 for our free service).

These terms are governed by the laws of the State of Delaware, United States. Any disputes will be resolved in the courts of Delaware.
